Evidence:Not supported

Our verdict: Not supported by clinical guidelines for hot flashes.

Black cohosh (Actaea racemosa / Cimicifuga racemosa) is one of the most heavily marketed botanical supplements for hot flashes and night sweats. However, in its 2023 Nonhormone Therapy Position Statement, The Menopause Society concluded that over-the-counter supplements and herbal remedies are not recommended for vasomotor symptoms (Levels I–II) because randomized controlled trials have not shown consistent benefit over placebo.

Why people experience relief in unblinded use (the placebo effect)

In menopause clinical trials, placebo groups routinely experience a noticeable reduction in hot flash frequency simply from tracking symptoms and participating in a study. When black cohosh is tested head-to-head against a matching placebo pill in large, independent double-blind trials, it does not reliably outperform placebo. Furthermore, over-the-counter botanical extracts vary widely in purity and standardization.

Because our evidence grade for black cohosh is Not supported, MenoVerdict never places affiliate links for black cohosh supplements on this page. See how we grade evidence and does it work? directory.

What has proven evidence instead

If you want to manage hot flashes without hormones, explore the Level I–II options recommended by The Menopause Society:

  • Since herbal supplements like black cohosh are not recommended by guidelines, which Level I hormonal or non-hormonal treatments fit my health history?
  • Could any over-the-counter supplements I am currently taking interact with my prescribed medications or affect my liver?

Questions people ask

Does black cohosh beat placebo for hot flashes in clinical trials?

Across higher-quality randomized controlled trials reviewed by The Menopause Society (2023), herbal remedies including black cohosh did not demonstrate consistent benefit over placebo for reducing hot flashes, leading the panel not to recommend them (Levels I–II).

What non-hormonal options do have strong evidence for hot flashes?

The Menopause Society (2023) recommends cognitive behavioral therapy (CBT), clinical hypnosis, SSRIs/SNRIs, gabapentin, oxybutynin, and neurokinin receptor antagonists such as fezolinetant.
